Care Pathways Coach (1 Position Alexander City, AL and 1 Position Montgomery, AL) – needed to work with nursing home to reduce avoidable hospital admissions for long-term stay medically complex beneficiaries and improve their care management, outcomes, quality, cost effectiveness and coordination of care. Requires: current unrestricted AL RN license; critical thinking skills; LTC, sub acute, LTAC, hospital, HH and/or hospice exp.; knowledge of LTC quality indicators and measures preferred; good organizational skills, excellent communication skills; ability to conduct educational programs; work independently; proficient in Microsoft Office Suite. Will evaluate appropriateness, efficiency and effectiveness, and recommend improvements through on-site visits, meetings, phone consultation and abstraction of data in collaboration with the team and leadership. Must be able to function independently yet work effectively in team environment. Bachelor’s degree in clinical/healthcare related field required; Master’s or advanced degree preferred. 5+ years experience with immunizations/vaccinations preferred. Alabama Registered nurse unrestricted license preferred. Advanced levels skills in Microsoft Suite, strong communication and organizations skills required.
