Project Coordinator needed as part of the Nursing Facility Initiative team which strives to reduce avoidable hospitalizations among nursing facility residents in participating nursing homes in the State of Alabama. This position will provide support, including secretarial and clerical, to participating stakeholders and other staff in all aspects of the project. The Coordinator will help plan and coordinate meeting/training events and other activities, and act as a resource for various software applications and programs. Qualifications are: high school graduate required, two years of college or associate degree preferred;
advanced level skills in Microsoft Office Suite and basic knowledge of Microsoft Project required; ability to work independently and as part of a team required; must be able to multi-task and problem solve. Knowledge and understanding of data and statistical reporting preferred; two years experience in administrative support preferred; current experience in a health care related field preferred (within last 5 years).

Care Pathways Coach (1 Position Cordova, AL and 1 Position Cullman, AL) – needed to work with nursing home to reduce avoidable hospital admissions for long-term stay medically complex beneficiaries and improve their care management, outcomes, quality, cost effectiveness and coordination of care.
